Novel Investigational Tools for Diabetes Mellitus

As understanding of the underlying pathophysiology and metabolic abnormalities leading to the development of type-2 diabetes mellitus has increased, novel pharmacologic approaches have been developed to better manage this disease. Since improved glycemic control reduces long-term microvascular complications, new peptide-based agents that act on insulin secretion and regulation have been used. Incretins are insulinotropic peptides that augment insulin secretion and suppress glucagon secretion. Glucagon-like peptide 1 (GLP-1) is the incretin with dominant biological activity, which is produced by the L cells of the intestinal mucosa from the same prohormone as glucagon.

Exendin-4, a GLP-1-like peptide from the Gila monster venom, is a naturally occurring long-acting GLP-1 receptor antagonist. An exendin-4 product (Exenatide) has been FDA-approved for type-2 diabetes treatment in controlling glycemic response and inducing weight loss.

Amylin                             Antibodies                Peptides 
Amylin is a 37-aa peptide co-secreted with insulin in response to meal ingestion. Pramlintide is a synthetic analog of amylin approved by the FDA as adjuvant therapy in type-1 and type-2 diabetes, acting by decreasing appetite, slowing gastric emptying and glucagon secretion.
