Export Licenses
In order to avoid delays in the processing of your Export License statement, for all product orders that require an Export License (#’s 100B, 104, 122, 123, 126A , 128A/C, 130A/B, 136A/B, 138A/B, 140A/B, 141A, 146A, 162L, 611, 612, 613A, 620A, 622A, 623A, 625A, 630A), the following sections in the Export License statement form must be completed:
- Section # 1: Ultimate Consignee
- Section # 2: Ultimate Consignee’s Authorized Recipient(s)
- Section # 3: Intermediate Consignee ( if the Ultimate Consignee and the Distributor’s office are located in the same country; Distributors are required to complete this section)
- Section # 4: Intermediate Consignee’s Authorized Recipient ( If the Ultimate Consignee and the Distributor’s office are located in the same country; Distributors are required to complete this section)
- Section # 5: Purchaser (All Distributors as the purchasers must complete section # 5)
- Section # 6 and 6a: Will the product(s) be re-exported?
- Section #7: List all products for which you are requesting an export license for an specific Ultimate Consignee. This can be for a current or future order.
- Section # 8: Specific end-use description: Ultimate consignee is required to provide a detailed description of the intended use.
- Section #11: Ultimate Consignee Signature
- Section # 12: Purchaser Authorized Signature
List Labs is unable to process an Export License request when any of the information listed above is missing resulting in orders processing delays.
Letter of Assurance
In order to avoid delays in the processing of your Letter of Assurance, for all product orders that require a Letter of Assurance (#’s 122, 123, 128A/C, 130A/B, 136A/B, 138A/B, 140A/B, 141A, 146A), the following information must be provided as attachment(s) with a completed and signed Letter of Assurance:
- A brief description of the research or testing which will be done using List Labs’ products. Justify the legitimate need.
- References for up to ten scientific publications and/or patents (particularly your own publications and/or patents) illustrating the receiver’s research interests, experience and capabilities.
- A short description of the institution and facility where work will be done.
The Letter of Assurance Appendix III form must be signed by the End User and PI or Institution Director.
List Labs is unable to process a Letter of Assurance when any of the information listed above is missing resulting in orders processing delays.
Purchase orders for export regulated and Select Agents & Toxins products
All purchase orders (PO) for products that require an Export License and/or a Letter of Assurance must include the following information:
- Name of the approved Ultimate Consignee or End User.
- Approved shipping address for the Ultimate Consignee or End User.
